Creating Strong Diabetes Programs: Plan a Trip to Success!

Want help in improving your Diabetes Program? This training and corresponding workbook offer an introduction to the concepts and processes of planning and evaluating a diabetes program. Continuing Education credits (7/14/2009 - PDF - 66KB) are available. Accredited Sponsors: IHS Clinical Support Center and IHS Nutrition and Dietetics Training Program.
For this training, please use the following steps: Ask team members to set aside at least one-half day. You may want to spread this training over a week or two. Print the Workbook (7/13/2009 - PDF - 616 KB) In the workbook, read "How to Use this Workbook" and "Introduction". Each section has one or two videos that introduce the topic and show a diabetes team working through the section. The Essential Elements provide detailed information while the Quick Facts provide the key points. Use the Action Steps to fully complete each section. When you complete the training and workbook, you will have a completed planning and evaluation project for a diabetes program.
